Michigan Medicine Laboratories now offers easy and convenient COVID testing for pre-procedural, symptomatic, and exposure indications. By appointment only.

	PROCEDURAL TYPE Nasopharyngeal Swab collected by staff* OR Self Collected Mid-Nasal Swab
	BILLING & COVERAGE Patient’s insurance will be billed. Uninsured patients will be tested at no cost.

*Michigan Medicine recommends a nasopharynegal swab collected within 72-96 hours of scheduled procedure for patients.

2. Sign and return a Fax Verification form authorizing delivery of secure COVID test results.
    
3. MLabs will provide you with custom test requisitions and instructions for your patients, students, employees or travelers.
