NEW PEDIATRIC GFR TEST COMPONENT
Creatinine, Serum
Order Code: CREAT
Fee Code: 20504 (CPT 82565)
Effective March 4, 2025, the MLabs Chemistry Core Laboratory will begin reporting an estimated pediatric glomerular filtration rate (PGFR), calculated using the CKiD Under 25 (U25) GFR equation to aid with diagnosis of chronic kidney disease in children. The PGFR will be provided for patients aged 12 months – 17 years and will be based on height and measured creatinine. The equation uses age and sex as specified.
For Michigan Medicine test orders, the most recent Height (collected within the previous 6 months of testing) and Date of Height recorded in the Michigan Medicine MiChart EHR will be used for the calculation of PGFR. For MLabs test orders or if no recent height or no height is recorded in MiChart, the height used for the calculation will be imputed from CDC growth charts using the 50th percentile for age and sex. Currently, the Chemistry Core Laboratory provides an estimated GFR for patients aged 18 years and older and will continue using a calculated CKD-EPI creatinine equation for that age group. All orders containing a serum creatinine will report the estimated glomerular filtration rate.
The following non-orderable test components will be added to Serum Creatinine (CREAT) and all panels that include Serum Creatinine (RENAL, BASIC, COMP) for patients aged 12 months – 17 years:
Group: PGFR Pediatric GFR
Components: HT Height for Pediatric GFR
DOHT Date of Height for PGFR
GFRP Pediatric estimated GFR
