Syphilis reverse algorithm is used to screen patients for syphilis. Positive or equivocal result will reflex to RPR and TPPA, if indicated to help differentiate between active infection, past infection, no infection or false positive total antibody. All results must be reviewed before making a clinical decision.
Nonreactive

A nonreactive result does not exclude the possibility of exposure to or infection with T. pallidum. Antibodies may be at low or undetectable levels in incubating or early primary disease and in some clinical conditions.
Detection of treponemal antibodies may indicate recent, past or successfully treated syphilis infections and therefore cannot be used to differentiate between active and cured cases.
A patient with a Syphilis Total reactive result will usually remain reactive for life, and therefore, antibody indices cannot be used to determine responses to therapy. In these cases, an RPR should be used to monitor the patient.

All reactive and equivocal results will reflex to an RPR and TPPA, if indicated. By ordering this test the clinician acknowledges that additional reflex testing will be performed and billed at a separate additional charge if indicated.
