Testing is performed on the patient's original NBS card. DO not collect a new sample for this order. Parents must request the spots be sent to Michigan Medicine. See the steps in Special Handling as to what is needed.

Special Handling

The following steps need to take place:
Parents or clerk will need to call MDHHS Newborn Screening Follow-up Program
Contact Carrie Langbo, who is in charge of the bio bank to obtain the specimen. Her number is 517-335-6497.

MDHHS will need the attached form to be completed and sent back to them, along with:
Birth hospital and Mom’s first and last name at delivery. (probably on birth certificate)
The attached form completed and returned (by fax or regular mail) – to Carrie. You may need to contact her for a fax number.
A copy of either the mother or father’s ID. This can be captured via photo on smart phone and emailed.
A copy of the child’s birth certificate (this can be the hospital issued or state issued certificate, whatever is on hand). This can be captured via photo on smart phone and emailed.
