Test Overview
Short term cell culture and chromosome analysis, cultures with and without mitogens. Fluorescence In Situ Hybridization (FISH) when indicated. Samples with an indication of mature B-cell lymphoma including, but not limited to, CLL/SLL, follicular lymphoma, mantle cell lymphoma and marginal zone lymphoma, will have an extra culture inoculated with CpG-oligonucleotide (CpG) and Interleukin-2 (IL2). Similarly, samples with an indication of mature T-cell lymphoma/leukemia (T-PLL, AITCL, ALCL) will have an extra culture inoculated with Phytohemagglutinin (PHA). A minimum of 10 metaphase cells are analyzed from mitogen stimulated cultures, in addition to the standard 20 metaphase cells analyzed from the unstimulated cultures.
Rule out acquired chromosomal abnormalities associated with hematologic disease. If the patient has at least 10% blasts (or a combination of blasts, promyelocytes and myelocytes totaling 10%) a peripheral blood sample may be useful, however, a bone marrow sample is preferred.

Specimen Requirements
Specimen transport should be arranged so that the specimen is received by MLabs the same day it is collected. Collect specimen in a dark green top (sodium heparin) tube. Invert the tube several times to prevent clotting. Send the specimen at room temperature as soon as possible. DO NOT CENTRIFUGE. Clotted samples, samples containing <5 million dividing cells, and samples that are greater than 24 hours old are considered suboptimal.

Appropriate FISH testing will be performed at an additional charge when indicated. FISH testing is available for a number of oncology probes, including BCR::ABL, PML::RARA, ETV6::RUNX1, and KMT2A and MDS FISH panel (see Cytogenetics, FISH for Malignancy).
