Effective immediately, all test requisitions must include the date and time of specimen collection. This information is essential for accurate testing, timely processing, and compliance with regulatory and accreditation standards. Failure to include the collection date and time may result in delays in processing or rejection of the specimen. Clearly write the collection date and time on the test requisition form.
