Test Overview

Test Methodology

Short term cell culture and chromosome analysis, cultures with and without mitogens. Fluorescence In Situ Hybridization (FISH) when indicated. Samples with an indication of mature B-cell lymphoma including, but not limited to, CLL/SLL, follicular lymphoma, mantle cell lymphoma and marginal zone lymphoma, will have an extra culture inoculated with CpG-oligonucleotide (CpG) and Interleukin-2 (IL2). Similarly, samples with an indication of mature T-cell lymphoma/leukemia (T-PLL, AITCL, ALCL) will have an extra culture inoculated with Phytohemagglutinin (PHA). A minimum of 10 metaphase cells are analyzed from mitogen stimulated cultures, in addition to the standard 20 metaphase cells analyzed from the unstimulated cultures.

Test Usage

Identification of specific chromosomal abnormalities associated with hematologic disease. Bone Marrow specimens are most commonly sent as part of the evaluation of acute leukemia (lymphoid or myeloid), MDS, or myeloproliferative disorders including chronic myelogenous leukemia, in the latter case to look for a Philadelphia chromosome.

Reference Range

Specimen Requirements

Collection Onsite

Chromosome Media tube. Sterile syringe rinsed with sodium heparin or green top vacutainer with sodium heparin. All pediatric bone marrow specimens and bone core biopsies must be transported in Chromosome Media available from Specimen Processing. Do not allow specimen to overheat or freeze. Clotted samples, samples containing <5 million dividing cells, and samples that are greater than 24 hours old are considered suboptimal. Please complete a MiChart test request and submit it with the specimen. A Cytogenetics Laboratory Requisition is available online at:….

Collection Offsite

Specimen transport should be arranged so that the specimen is received by MLabs the same day it is collected. Collect bone marrow using aseptic technique. Place 1 - 3 mL bone marrow aspirate in a green top (sodium heparin) tube. Invert the tube several times to prevent clotting. Send at room temperature as soon as possible. Bone core biopsies must be transported in sterile transport media. Do not allow specimen to overheat or freeze. Clotted samples, samples containing <5 million dividing cells, and samples that are greater than 24 hours old are considered suboptimal.

Normal Volume
2-3 mL bone marrow
Minimum Volume
1 mL bone marrow
